Please note: The USCIS is now requiring applicants to be fully vaccinated for COVID-19 starting October 1, 2021.

What to Bring:

Form I-693, Report of Medical Examination and Vaccination Record. The medical professional who completes your exam will fill out this form and seal it in an envelope for you to submit to USCIS. If you do not have Form I-693, we can provide it for you.

Photo ID

Photo ID is needed for all ages.

These physical exams must be done by a doctor who has been authorized by the U.S. Citizenship and Immigration Services. What do they check in an immigration physical?


The immigration physical exam will check on your medical history, immunization history, and current health. The doctor will do blood tests to check for communicable diseases like tuberculosis, syphilis, and gonorrhea. These diseases are a public health risk. Children under 15 are normally not tested for syphilis or gonorrhea. The doctor will check for mental well-being, especially mental illnesses that are related to harmful or violent behaviors. You will also be examined for signs of drug abuse or addiction.
 
If you are between 18 and 64 years old, you will require immunizations/vaccines against tetanus, diphtheria, measles, mumps, rubella, varicella, hepatitis B, influenza (flu), and covid-19. Pregnant women may not need to have some of the vaccines.
 
Immigration physicals are different from standard physical exams. The purpose of the immigration physical is to screen for specific medical conditions that are considered public health and safety concerns by the U.S. government. The doctor does not need to examine or treat patients for diseases or medical conditions that the U.S. government does not deem relevant.

What should I bring to the immigration physical?

We will provide you with Form I-693 (Report of Medical Examination and Vaccination Record). The doctor will use this form to record the results of your physical. If you happen to have the form with you before you arrive, you may complete pages 1-4.
 
Other items that you need to bring:
Government-issued photo identification, such as a passport, state ID, travel permit, driver’s license, or work permit.
 
Your immunization or vaccine records. You must have proof of immunizations. If you do not have proof of immunizations, we will provide you with information on where to get it.
